Office Hours

We see patients by appointment from 8:30 am to 4:30 pm, Monday through Thursday and 8:30 am to 11:30 am on Friday.

However, we have people in the office to answer phones from Monday through Friday 8:30 am to 4:30pm

Urgent or emergency appointments may be scheduled on Friday afternoons.

Making the Most of Your Visit

  • We realize that your time is valuable and try our very best to stay on schedule. We ask you to assist us by being on time for your appointment.
  • Please bring your current insurance card, photo ID and payment for any applicable copay to each appointment.
  • Minors must be accompanied by a parent or guardian. If accompanied by another adult, they should bring written permission giving authorization to treat in the presence of the responsible individual. Please check out our forms tab for our minor consent form.  You can print that off and have your child bring it to their appointment.
  • Let us know when scheduling your appointment if you are in need of interpreter services for your visit. We will gladly set up an interpreter for your visit so you can get the most out of your appointment with our specialists.
  • Please cancel 24 hours in advance if you cannot keep your scheduled appointment so that your appointment time will be available for other patients.
  • We ask that you please limit the amount of perfume/body spray that you wear when visiting our office. Many of our patients visit us for sensitive allergy issues, and this can cause flare ups and discomfort.  Thank you for your understanding.

New Patients

Please, come at least 15 minutes prior to your appointment.  Don't forget to bring your current insurance card, photo ID and payment for any applicable copay to your appointment.

Before seeing one of our specialists, many of our patients have undergone studies and/or treatments related to their current problem. To allow us to make the best use of your visit, please bring with you the following:

  • A list of your current medications with dosages.
  • A list of any medications that have been used to treat your current problem in the past, even if you are no longer taking them or if they were ineffective.
  • Any records from other physicians or studies you have had related to the medical issue you are being seen for.

Dizzy Patients: If you are a new patient being seen for dizziness we ask you to also complete the Dizziness Questionnaire under the FORMS section.  PLEASE ARRIVE 15 MINUTES EARLY TO ALLOW ENOUGH TIME TO COMPLETE YOUR PAPERWORK.

No Show/Cancellation Policy:

Our goal is to provide our patients with high quality care in a timely manner. With a current up rise in the number of patients who are not showing for their scheduled appointments, we have decided to implement a cancellation/no show policy.  We understand that in certain situations, you must cancel your appointment.  Please call to cancel at least 24 hours in advance.  This allows adequate time to fill your appointment slot with another patient who needs accesss to our physicians and staff.

Patients who do not show up for their appointment, without a call to cancel, will be considered a NO SHOW.  This will be documented in our system, and if you need to call and make another appointment, you will be charged a $50 fee, to be paid in full, before being allowed to make that appointment.  This will not be covered by your insurance.

Patients who do not show for their appointment(s) 2 or more times, will be considered for dismissal from the practice.  This will be at the discretion of the provider, and dismissal will result in denial of any future treatment/appointments by our practice.
