How to Get a Referral
Contact your Primary Care Provider (PCP) before scheduling a specialist appointment.
A referral is not required for:
• Audiologists
A referral is required for:
• Otolaryngologists (ENT)
• Otolaryngologists (Allergy)
Referral Details:
• Covers up to 6 visits
• Valid for 6 months
Avoid delays in care — check with your PCP today if you need a referral to see an ENT Specialties doctor!
NO-SHOW FEE
Effective 03/26/2026 surgical appointment cancellations must be made at least 5 business days prior to your scheduled surgery, or a $300 no-show fee will be charged.
