Office Hours

We see patients by appointment from 8:30 am to 4:30 pm, Monday through Thursday and 8:30 am to 11:30 am on Friday.

However, we have people in the office to answer phones from Monday through Friday 8:00 am to 5:00 pm

Urgent or emergency appointments may be scheduled on Friday afternoons.

Making the Most of Your Visit

  • We realize that your time is valuable and try our very best to stay on schedule. We ask you to assist us by being on time for your appointment.

  • Please bring your current insurance card, photo ID and payment for any applicable copay to each appointment.

  • Minors must be accompanied by a parent or guardian. If accompanied by another adult, they should bring written permission giving authorization to treat in the presence of the responsible individual.

  • Let us know when scheduling your appointment if you are need of interpreter services for your visit. We will gladly set up an interpreter for your visit so you can get the most out of your appointment with our specialists.

  • Please cancel 24 hours in advance if you cannot keep your scheduled appointment so that your appointment time will be available for other patients.

New Patients

Please, come at least 15 minutes prior to your appointment.  Don't forget to bring your current insurance card, photo ID and payment for any applicable copay to your appointment.

Before seeing one of our specialists, many of our patients have undergone studies and/or treatments related to their current problem. To allow us to make the best use of your visit, please bring with you the following:

  • A list of your current medications with dosages.
  • A list of any medications that have been used to treat your current problem in the past, even if you are no longer taking them or if they were ineffective.
  • Any records from other physicians or studies you have had related to the medical issue you are being seen for.

Dizzy Patients: If you are a new patient being seen for dizziness we ask you to also complete the Dizziness Questionnaire under the FORMS section.  PLEASE ARRIVE 15 MINUTES EARLY TO ALLOW ENOUGH TIME TO COMPLETE YOUR PAPERWORK.

No Show/Cancellation Policy:

Our goal is to provide our patients with high quality care in a timely manner. With a current up rise in the number of patients who are not showing for their scheduled appointments, we have decided to implement a cancellation/no show policy.  We understand that in certain situations, you must cancel your appointment.  Please call to cancel at least 24 hours in advance.  This allows adequate time to fill your appointment slot with another patient who needs accesss to our physicians and staff.

Patients who do not show up for their appointment, without a call to cancel, will be considered a NO SHOW.  This will be documented in our system, and if you need to call and make another appointment, you will be charged a $50 fee, to be paid in full, before being allowed to make that appointment.  This will not be covered by your insurance.

Patients who do not show for their appointment(s) 2 or more times, will be considered for dismissal from the practice.  This will be at the discretion of the provider, and dismissal will result in denial of any future treatment/appointments by our practice.

Patient Portal Information

All patients will be assigned a link to our Patient Portal when scheduling their appointment. The Patient Portal offers you the ability to review your visit sumary(s) and email us. Although the portal allows you to email us about anything, we ask that for rescheduling appointments, you contact our Front Desk at (402) 488-5600 during our office hours. 

If you have given us your email address, you will receive a link for the patient portal in your inbox. In the email, you will receive your log-in information and passowrd. You need to remember these once you click on the link. After entering the ID and password, you will be prompted to change your password. Set it to whatever you would like it to be as long as it meets all of the security requirements. It will then ask you for a 4-digit PIN. This is your YEAR OF BIRTH (i.e., 1987). You will then be asked to answer all of the questions about your health history and current medications. On the final page, you must click "SUBMIT". If you do not, the EHR will not register your information, and you will be asked to fill out a form in the office. 

Without an email address, we can still give you a username and password for the patient portal. We can give you a printed instruction guide before or after your visit with us. When you log in, you will have to repeat the steps listed above for reseting your password and using your PIN. A link to the Patient Portal is provided below.
